What Is A Facelift?
A facelift, also known as a rhytidectomy, is a facial plastic surgery procedure that can turn back the clock and transform a person’s life, but not all facelifts are the same. Many plastic surgeons still use the old-fashioned approach, which simply pulls the facial skin back and tightens it up, sometimes creating a distorted and windswept look. Furthermore, the results from this older approach only last a year or so, after which the skin begins to loosen back to where it was.
To prevent this, Dr. Thomas Romo III, top facelift surgeon in NYC, developed a unique facelift technique called the endoscopic composite facelift, honed over his many years of experience. This approach has numerous advantages over other facelift techniques, including more natural-looking and long-lasting results, smaller incisions, and more rapid recovery after surgery.
What Concerns Can A Facelift Treat?
A facelift can:
- Tighten and rejuvenate sagging facial skin
- Fix separation that may have developed between the lower eyelids and cheeks
- Get rid of deep creases along the nose that extend to the corners of the mouth (also known as nasolabial folds or laugh lines)
- Address facial fat that may have fallen or disappeared over time
- Eliminate jowls created from loss of skin tone in the lower face
- Address loose skin and excess fatty deposits under the chin and jaw

Will A Facelift Improve My Neck And Jaw?
Yes. Facelift techniques primarily focus on the lower two-thirds of the face. The repositioning of muscle, fat, and skin across the cheekbones can often reduce the appearance of jowls that soften the jawline. Additionally, a mid-face lift smoothes the nasolabial folds that deepen around the nose and mouth as the volume is lost in this area.
For more extensive tissue laxity at the jawline and neck area, a combination facelift and neck lift may be more appropriate. A full consultation and examination are performed with each patient to understand the extent of aging and their desired outcome from treatment.

Can I Have Facelift Surgery If I Already Had A Facelift In The Past?
Facelift surgery can make a person appear years younger. The results last indefinitely when compared to how the face would look without any intervention. However, aging is a progressive aspect of life that will continue, facelift or not. For this reason, some people do choose to undergo a second (or third) facelift in time.
It is possible to undergo a facelift if you’ve already had treatment in the past. Some people do this after as little as five years. while others seek additional rejuvenation many years after an initial facelift. With so many options to address the signs of aging, the timing of facelift surgery can vary widely from one person to another.

Is A Facelift/Neck Lift Appropriate For Men?
In recent years, the number of men undergoing facial rejuvenation procedures has increased substantially. Men can and do benefit from procedures such as facelift and neck lift surgery.
- One study indicated that men looked more attractive and trustworthy after a facelift.
- The same study indicated that men looked more masculine and outgoing after a neck lift.
- Neck lift surgery is among the most popular among men.
How Does The Face Age?
Candidates For Facelift Surgery
A facelift may be recommended for patients of both genders facing one or more of the following aesthetic concerns in the face:
- Loose or sagging skin that ages the face
- Visible signs of aging in the face
- Lax facial muscles
- Deep creases in the face
- Excess fat deposits
- A jawline which lacks contour
Can Facelift Surgery Be Combined With Other Procedures?
Facelift surgery alone focuses on issues that develop in the lower part of the face, from the jaw to the cheek area. Depending on the extent of volume loss due to bone and fat pad decrease, a patient may benefit from a combination of treatments. Examples include blepharoplasty eyelid rejuvenation, brow lift surgery, neck lift surgery, and injectables.
Depending on a patient’s needs and desired results, Dr. Romo may also recommend a neck lift or an eyelid surgery (blepharoplasty) in conjunction with a facelift to achieve the best and most natural-looking results.
Types Of Facelift Procedures
Several different terms may be used to describe a facelift procedure. These include full facelift, mid-facelift, lower facelift, SMAS lift, deep plane facelift, and more. Generally speaking, facelift surgery is a procedure in which the jawline and cheek areas are improved. Incisions are made in the area in front of and around the ears to gain access to the muscle, fat, and skin on the lower two-thirds of the face. The repositioning that occurs during surgery improves contours along the jawline, the chin, and the cheeks. Facelift surgery does not affect the upper one-third of the face. For rejuvenation of the eyes and forehead, a surgeon may suggest blepharoplasty or a brow lift.
The Facelift Procedure
Dr. Romo’s endoscopic facelift procedure utilizes a sequence of surgical techniques that produce a natural, youthful and rejuvenated appearance with smaller incisions and less downtime.
Here’s an overview of what you can expect during a facelift:
- A facelift can take 2-3 hours, depending on the complexity
- When performed in conjunction with other procedures, such as a neck lift, the surgery time will vary
- You will be given an anesthetic so you will not feel any pain during the procedure
- After surgery, you may have some swelling, bruising and tenderness
- Recovery and potential downtime can range from 10‒14 days
- Incisions are small and placed strategically in order to conceal the minimal scarring
- When a facelift is performed in combination with a neck lift, incisions are typically made along the ear creases and into the hairline to conceal any minimal scarring
Recovery From Facelift Surgery
Facelift recovery typically progresses over about two weeks. During the first week, rest and pain management are top priorities. By day 3 or 4 after surgery, swelling and bruising typically peak. Within the first 7 days, patients typically transition from needing prescription pain medication to managing comfort with over-the-counter medication. Between days 4 and 7, it is safe to say patients will feel better than they look.
Bruising and swelling, along with other sensations like tingling and numbness, are the primary side effects experienced from day 7 to 14 of recovery. By the end of the second week, patients are often ready to return to work and most other normal activities. Weeks two to four after facelift surgery include continued improvement in sensation and contouring. Swelling and bruising may still be present, and incisions may appear slightly pink. However, energy may be largely returned to normal and patients may begin light exercise. Sutures are removed sometime between weeks one and three after a facelift. By the end of the fourth week of recovery, much of the improvement that has been achieved is apparent.
